Dentists are trained to diagnose what is in front of them. Running a practice is harder because some of the most expensive problems do not announce themselves.

You can be busy and still be leaking money. You can have a full schedule that looks impressive until you notice how much of the day is tied up in low-value procedures, late cancellations, poor scheduling decisions, and treatment that never gets back onto the books. Everyone feels slammed. Production says otherwise.

The same thing happens with marketing.  You search “super best gift-from-god dentist in my town” and, Voila, your practice appears at the top, and you pat yourself on the back for making the website all by your amazing self… Or the hubby did it… Or that “patient who does websites”. Except, what you don’t realize is that Google is showing that result because the search is coming from the same office, the same home, the same devices, with the same browsing history. Google keeps offering you the most frequent site you visited… Your own!

Meanwhile, the dentist three miles away is the one patients are actually finding at the top of Google’s first page.

Remember this… Blind spots are expensive.

An office manager can be loyal, hardworking, and still be ineffective. Those things are not mutually exclusive. If the schedule is constantly breaking down, staff problems keep landing on the doctor, collections are slipping, and policies depend on who is working that day, the title on the business card is not the issue. The result is.

Collections are another place where comfort can hide trouble. Money is hitting the bank account, so things feel normal. Then somebody actually looks at receivables by age, unresolved insurance claims, patient balances, unexplained write-offs, and accounts that have been sitting untouched for months. “We are collecting well” can fall apart very quickly when the numbers are examined instead of admired.

And staff members are very good at adapting systems to suit themselves. Not maliciously. Usually gradually. A step gets skipped because it seems unnecessary. A policy becomes optional because enforcing it creates friction. Somebody else decides a task belongs to another department. Six months later, the practice is operating according to a set of unwritten rules nobody intentionally created.

The same goes for treatment. The doctor diagnoses it. The patient says he wants to think about it. Then the treatment plan disappears into the software. Nobody calls. Nobody revisits the objection. Nobody tracks what was never scheduled. There may be thousands of dollars in needed dentistry sitting there, but because it is not visible on today’s schedule, it barely exists.

This is why an outside review matters.

Not because every practice is dysfunctional. Most are not. But owners are too close to the daily operation to see everything clearly, and vendors and employees naturally report from their own point of view. Somebody has to look at the practice as a business, follow the money, test the assumptions, and find out where the doctor is being told “everything is fine” when it really is not.
